Migraines are not simply severe headaches. They are a complex neurological disorder involving abnormal brain activity, changes in blood flow, and activation of the trigeminovascular pathway, the network of nerve fibers and blood vessels that, when sensitized, generates the intense, often debilitating pain and accompanying symptoms of a migraine attack.
Migraine attacks typically progress through four phases. The prodrome phase, which can occur hours to days before the headache, may involve subtle warning signs such as mood changes, food cravings, neck stiffness, or fatigue. The aura phase, which affects roughly a third of migraine sufferers, involves neurological symptoms such as visual disturbances, tingling, or speech difficulty. The headache phase brings the characteristic throbbing or pulsating pain, often on one side of the head, accompanied by nausea, vomiting, and extreme sensitivity to light and sound. The postdrome phase, sometimes called the migraine hangover, can leave patients feeling drained, confused, or physically depleted for hours after the pain subsides.
Chronic migraine is defined as 15 or more headache days per month, with at least eight of those meeting migraine criteria. Even episodic migraine, typically defined as fewer than 15 headache days per month, can severely impair work performance, relationships, and quality of life when attacks are frequent or severe. For patients in this category, particularly those who have not responded adequately to standard pharmacological approaches, rTMS represents a meaningful alternative worth serious consideration.
Repetitive transcranial magnetic stimulation uses focused magnetic pulses delivered through a coil placed against the scalp to modulate activity in targeted regions of the brain. For migraines, the primary targets are the motor cortex and, in some protocols, the dorsolateral prefrontal cortex. Research indicates that high-frequency rTMS applied to the motor cortex can reduce cortical excitability in a way that may interrupt the cascade of neurological events that trigger and sustain migraine attacks.
One mechanism of particular interest is rTMS’s potential to disrupt cortical spreading depression, the wave of neuronal and glial depolarization that is closely linked to migraine aura and to the onset of migraine pain. By modulating the excitability of cortical tissue, rTMS may reduce the frequency with which this cascade is triggered, offering a preventive rather than merely symptomatic effect.
A randomized controlled trial examining multifocal rTMS in episodic migraine found that real rTMS reduced mean migraine days per month by more than three days compared to sham treatment over a 12-week period, with a meaningfully higher responder rate among those receiving active stimulation. A broader meta-analysis combining data from multiple randomized controlled trials found that rTMS was more effective than sham stimulation for decreasing migraine attack frequency, with an effect size indicating clinically meaningful improvement.
rTMS is not a cure for migraines, and the evidence base continues to grow. It is best understood as a promising neuromodulation option for patients who have not found adequate relief through conventional preventive or abortive therapies, or who wish to reduce their reliance on medications. Your first appointment will begin with a comprehensive clinical evaluation by one of our board-certified psychiatrists or psychiatric mental health nurse practitioners. Your provider will review your migraine history in detail, including frequency, severity, duration, triggers, associated symptoms, prior treatments, and any relevant medical or psychiatric history. This evaluation ensures that rTMS is appropriate for your situation and informs the stimulation protocol your treatment team will design.
rTMS sessions at Complete Mind Care are outpatient, typically lasting 20 to 40 minutes, and require no anesthesia, sedation, or recovery time. You can drive yourself to and from appointments and return to your normal activities immediately after each session. A standard rTMS course for migraine typically involves daily sessions five days per week over four to six weeks, though your protocol may vary based on your clinical picture and your response to treatment.
The most commonly reported side effect is a mild tapping or discomfort sensation on the scalp during stimulation, which most patients find decreases significantly after the first few sessions. rTMS has no systemic side effects and does not cause the weight gain, cognitive dulling, or cardiovascular concerns associated with many oral migraine preventives.
